Falsification of any of the above information will be grounds to disallow your adoption of a rescued dog. Please use your typed name as your signature. I understand that all domesticated animals MUST be up to date on ALL vaccines, spayed/neutered and current on Heartworm prevention to be considered for adoption. We do not adopt dogs/cats to outside homes and all animals must be housed inside. Please understand that applications can take several business days to process.
